Do you hold or have you ever held a Top Secret or higher security clearance?
Are you a member of any professional organizations, unions, or advocacy groups?
Do you frequently (more than 4 times annually) travel internationally?
Do you possess security-related training?
Section 2: Online & Public Presence
Do you own or rent your residence?
If owned, is the property registered under your legal name?
Are utilities registered in your name?
Do you own or operate any personal or professional websites?
Do you maintain any social media accounts?
Has your personal information been leaked on the dark web?
Have you experienced any other security breaches (e.g., hacking, identity theft, unauthorized access to personal/professional data)?
Have you been publicly mentioned in any high-profile, notable, and/or controversial media articles, social media posts (including but not limited to X), or other online sites such as the Watchlists of civil servants within the past 36 months?
Section 3: Threats & Risks
Have you or your family members been the target of threats, harassment, or stalking in the last 3 years?
Are you aware of any direct safety and/or security threats associated with you because of your current or former government role, or otherwise?

Have you ever been arrested, charged, or convicted of a crime?
Are you currently involved in any active or pending legal matter, and/or have you ever been involved in any civil litigation or lawsuits?
Have you been involved in any controversial legislation, public disputes, or high-profile legal actions?
